Impact of Low-Carbohydrate Diet (LCD) and Obesity Management Program on Obese Patients

Abstract: The past two decades witnessed a rapid escalation in the prevalence of obesity. Ayurvedic physicians use a multi-faceted strategy to treat obesity which includes a combination of Low-Carbohydrate Diet and Obesity Management Procedure. The present study was conducted to demonstrate the efficacy of low carbohydrate diet and the Ayurvedic obesity management procedure in reducing obesity. A retrospective observational study was conducted in Madhavbaug Clinics from September 2017 to March 2018. All patients with BMI > 30 kg/m2 were included in the study. During the study, the patients received seven sessions of obesity management procedure which included a combination of Snehana / external oleation or massage, Swedana / passive heat therapy and Basti kadha. Simultaneously, patients were prescribed supervised low carbohydrate diet daily for 90 days. The primary efficacy end point was reduction in body weight while secondary end points included changes in abdominal girth, systolic and diastolic blood pressure, heart rate after 90-day follow-up as compared to baseline. A total of 48 patients were enrolled and after screening, 31 were included in the study. Most of the patients were middle aged (51.35 ± 11.44 years) and females (65.8%). The primary end-point used in the present study (body weight) decreased significantly from 89.87± 18.58 kg on Day 1 to 82.01± 20.55 kg on day 90 (p<0.001). Such significant changes were also appreciable in the BMI, SBP, DBP, abdominal girth values measured on day 1 and day 90 respectively (p<0.001 for all). The present study demonstrates that the combination of low carbohydrate diet and Ayurvedic obesity management procedures are effective in reducing obesity.

Abstract: Introduction: We aimed to improve the chance of early diagnosis and curative treatment, to evaluate the level of knowledge about breast diseases and complaints, to increase the awareness and relationship between patients and surgeons, and to put forth the patient's expectations from surgeons because of the importance of breast cancer. Materials and Methods: 100 female patients, who were admitted to general surgery policlinic of Izmir University Faculty of Medicine between January 2016 to April 2016 with complaints of breast disease were studied prospectively via face-to-face questionnaire form. Results: The relationship between having knowledge about mammography and the frequency of mammography was examined and it has been statistically shown that patients who have knowledge about mammography have taken mammography more regularly and frequently. The relationship between complaints of admission to the general surgery policlinic and smoking was examined and it was statistically shown that complaints of the smokers were more frequent. Conclusion: Early diagnosis of breast cancer gives a chance to have a healthy life while at the same time enables breast preserving surgery with early diagnosis and increases the quality of life. It is necessary to accelerate the level of patient knowledge and awareness.

Abstract: Objective We conducted a public health risk assessment program for the Guangxi Zhuang Autonomous Region for typhoon disaster, to provide scientific evidence for public health emergency response and preparedness. Methods The public health risk assessment model for natural disaster is established through literature reviews, brainstorming, Delphi’s expert consultation and evaluation, based on theories of management, sociology and epidemiology. Basic data related to natural disasters and public health were extensively collected before the risk assessment. 34 public health experts from different professional fields identify the risk factors based on the basic data, conduct risk analysis according to the scoring criterion, calculate the risk values of risk factors according to the evaluation model, and then conduct the risk evaluation. Results 20 major public health risks which could seriously affect the Guangxi Zhuang Autonomous Region for typhoon disaster, were identified and assessed. The results of risk analysis showed that trauma, typhoid or paratyphoid, cholera, infectious diarrhea and hepatitis A were the top five high risk factors. Conclusion This study is the first time to apply the risk assessment theory for public health emergency preparedness before the natural disaster. Try to establish assessment index system, and then calculate risk value by semi-quantitative assessment method. This study provides a technical support for early warning surveillance and dynamic risk assessment of public health events. It will eventually improve capacity of public health emergency preparedness and response on natural disaster.

Abstract: Background: Harmful use of alcohol causes a large disease, social and economic burden to societies. Alcohol use is the third largest disease risk factor for deaths among youths between 15-29 years that contribute to 4% of the global burden of disease. Eastleigh also is known as “Little Mogadishu” is a suburb in Nairobi that is mostly inhabited by both Somali Community from Kenya and majorly from migrations from Somalia. It hosts about 30,000 Somali refugees and characterized by extremes of wealth and poverty. Objective: The aim of this study is to identify the prevalence of alcohol use and the patterns of use among Somali Youth living in Easteigh suburb in Nairobi. Method: This is an exploratory cross-sectional study among youth aged between 18-25 years of age that used a face-to-face interview. Results: Findings indicated a high prevalence among those who are male, of younger age, with married parents, born in Eastleigh, with lower educational level, the singles, the unemployed and those with high family income. Alcohol use is comorbid with other licit psychoactive substances that include tobacco products, miraa and shisha. Conclusion: These participants have relatively high alcohol use that is comorbid with other licit psychoactive substances. Recommendations include further studies to be done in this community using standardized instruments to cover a larger area and interventions to target younger youth of below 15 years of age and policy on treatment of young people with mental disorders to include substance abuse as well.

Abstract: There are only few clinical studies that address the effects of unemployment on somatic health. Only under certain, particularly strict criteria, are unemployed adults to be included in studies as they are subjected to special attention by ethics committees given their status as "vulnerable subjects". Our original goal was to obtain data on the risk factor unemployment via a pseudonymized survey and a fitness test (spirometry or ergometry). A problem we faced was that majority of participants were very concerned about answering questions on their state of health. Even with a modified study design, an entirely anonymous survey, the concerns remained. Subsequent expert interviews showed that unemployed people, as patients, even have reservations when talking about their health to their doctors. They fear that the health data stored in the social insurance data set was accessible to the AMS. This behavior of avoidance could affect the health care of this demographic.
