Epidemiology and Management of Orbital Cellulitis Complicated by Cerebrospinal Disease

Abstract: Orbital cellulitis is defined as an inflammation of the cellulo-fatty tissue of the orbit. Consecutive to an attack by an infectious agent, they are responsible for serious ocular, venous and especially cerebral complications. Cerebral or intracranial complications are responsible for 5 to 25% of mortality cases. The objective was to reveal the seriousness of cerebral complications of orbital cellulitis to contribute to better patient care in our context. Observation: We report three cases of orbital cellulitis received in ophthalmological consultation whose evolution was marked by the occurrence of cerebral complications which involved the functional and vital prognosis of patients. Discussion: Brain damage from orbital cellulitis is rare but remains the most serious complication. Computed tomography confirms the diagnosis. Their management is multidisciplinary and requires close collaboration between ophthalmologists, radiologists, otolaryngologists, and neurosurgeons. It must be rapid and adapted because the delay in consultations and patient care explains the evolution of the pathology towards these fatal outcomes. Orbital cellulitis is an ophthalmological emergency whose medical management should not suffer from any delay. Conclusion: Orbital cellulitis are rare but serious pathologies and responsible for many complications, particularly cerebral. Antibiotic therapy associated with rapid surgical management can considerably improve the vital and functional prognosis.

Abstract: The objective is to study the astigmatism induced after phacoalternative surgery in order to improve the quality of the surgery. This was a prospective and descriptive cross-sectional study carried out at the Donka University Hospital in Conakry from july 2021 to january 2022, i.e. a duration of 7 months, on 100 operated eyes. It involved 100 eyes, operated by phacoalternative with an upper limbal incision of at least 6 mm and implantation in the lens bag. Automatic refraction was performed pre-operatively and post-operatively at 45ᵉ day. The mean age was 62 years. Visual acuity was reduced to light perception in 58% of cases preoperatively. The mean implant power after biometry was 19±2.38 Dioptres (D); 50% of patients received the calculated implant. Corneal oedema was the most frequent early complication. Regarding the functional outcome, 82% of the eyes had an uncorrected visual acuity greater than 3/10 at D45 and 92% with correction. 95% of preoperative astigmatism was less than 2D and 57% according to the rule. The average postoperative astigmatism induced was 3.15D against the rule. In conclusion, phacoalternative surgery with a linear superior incision induces an astigmatism against the rule of 3.15 dioptres on average.

Abstract: Grey theory is a kind of grey system theory founded by Professor Deng Jurong, a Chinese scholar in the eighties century. It is a new method to study the uncertainty problem with few data and poor information. In recent years, grey modeling forecasting method has been widely used in industry, agriculture, science and technology, economic and social development planning and analysis, hydrology, geology, breeding and natural disaster prediction and so on. The prediction of epidemic trend of infectious diseases is an important work in the prevention and control of infectious diseases. Therefore, the author of this paper studied the application of grey modeling theory in the prediction and prediction of epidemic trend. Objective: To introduce the grey modeling prediction theory into the prediction of epidemic trend of infectious diseases, and to provide a new prediction method of epidemic trend of infectious diseases. The methods are as follows: The GM(1.1) model was established with the grey system theory, and the grey modeling was carried out on the short and medium term confirmed cases and the wave time of the epidemic outbreak in China in 2021, and the simulation accuracy and precision were analyzed. Results: The average simulation accuracy of the established short-term prediction model reached 73.2% and the prediction accuracy reached 99.85%. The average accuracy of the simulation value of the model reached 97.67% and the prediction accuracy reached 97.33% for the time of wave occurrence in the epidemic. This paper also analyzes the reasons for the low simulation value and prediction accuracy of the grey model forecasting method for the medium and long term confirmed cases of the epidemic.
